1. Entry to the Giveaway is available only to persons over the age of 18. You may be asked, at any stage, to provide Springboard, Inc. with proof of age and /or identity.
2. The Promotional Period shall end on October 31, 2020. The Giveaway drawing shall take place on or about November 6, 2020.
3. Each entrant shall be entitled to make a single entry throughout the Promotional Period. Entry into the Giveaway is free. In the event that Springboard, Inc. deems an entrant to have in any way acted in bad faith with respect to the Promotion, such entrant shall be excluded from the Giveaway and consequently deemed ineligible to win a Prize, as defined below.
4. In order to enter the Giveaway, each entrant shall submit complete wage survey responses, including, but not limited to name, email and phone number.
5. The Prizes for Giveaway shall be AirPods, a flight voucher, Cath Lab Academy Courses, EP Academy Courses, and 5 memberships to ACVP.
6. Each winner specifically acknowledges that winner shall be responsible for the payment of all tax, title fees, license fees, insurance requirements and other charges as may be required under any law, ordinance, statute, regulation or otherwise.
7. Springboard, Inc. shall notify each winner of the prize won via mail within ten (10) business days of the Giveaway Drawing.
8. Springboard, Inc. decision is final with respect to all matters relating to the awarding of the Prizes and shall not be subject to review or appeal by any entrant or by any third party.
9. Your participation in the Giveaway is deemed your agreement that Springboard, Inc. may use your name, likeness or image for promotional and marketing purposes without the requirement for any payment to you and you hereby grant Springboard, Inc. the right to do the same and expressly waive any claims against Springboard, Inc. in this regard.
10. The Prizes are neither transferable nor exchangeable and cannot be exchanged for money or money’s worth.
11. By entering the Giveaway, each entrant unreservedly agrees to these terms and conditions which govern the Giveaway and the awarding of the Prize.
12. By entering the Giveaway each entrant agrees to release, discharge and hold harmless the Springboard, Inc., its legal representatives, affiliates, subsidiaries, agencies and their respective officers, directors, employees and agents from any damages whatsoever suffered, sustained or allegedly sustained in connection the Giveaway or the acceptance of the Prizes.
13. The winning entrants shall be solely responsible for any taxes levied in relation to the delivery or receipt of the Prizes.
14. Springboard, Inc. reserves the right to alter these terms and conditions at any time and in its sole discretion.
15. Springboard, Inc. reserves the right, at any time, to cancel, modify or suspend the Giveaway if, in its sole judgment, the Giveaway is not capable of being conducted as specified.
16. These terms and conditions and any matters relating hereto shall be governed by and construed in accordance with the laws of Arizona and jurisdiction over any and all disputes shall be exclusive to the state and federal courts in Phoenix, Arizona.
